INSURANCE AND PAYMENT OPTIONS

We have found that many insurance policies do not cover every aspect of infertility testing and treatment. Therefore, the first step in assessing your potential financial obligation is a careful review of your specific insurance policy in relation to the treatment path that is determined at your initial physician visit. With this in mind, it is imperative that you bring or send each partner's insurance booklet(s) along with a copy of each partner's insurance card (front and back) prior to your initial visit. Please note this also applies to any prescription benefit booklets can be obtained at your employer's human resources office.

Due to frequent changes in insurance plan coverage, we are continuously evaluating participation with the various area insurance plans. Even if we are not participating with your particular PPO or POS plan, you may have insurance benefits that cover some or all of our services. You may phone our office to determine whether we are currently participating providers on your plan.

Due to the complexity of the insurance issues, payment is due at the time of service for many of the services that we provide. We will assist you with filing insurance claims. In these instances, filing your own insurance claims keeps down your cost of care while enabling you to maximally utilize your insurance benefits.

To help with the financial aspect of your treatment we accept Visa, MasterCard, American Express, Discover, debit cards, personal checks or cash. In addition, a payment plan (via third party financing) is available for many of the fertility procedures that are not covered by your insurance policy.

ACIRM also offers an IVF refund program (also known as "shared risk program" or "guarantee program") for those couples seeking this option.
